To earn CME credit, you must complete the following:
- Correctly answer at least 80 percent of the questions on the module
- Submit a short evaluation form of the online CME module process
- Fill out the certificate form to receive and print your CME credit certificate
Target Audience:
This activity is intended for pediatricians, family practitioners, other primary care physicians and allied health professionals involved in the care of the child who is a survivor of pediatric or adolescent cancer.
Objectives:
At the completion of the activity the participant will be better able to:
- List potential health problems (late effects) after childhood cancer therapy
- Identify which therapies put survivors at risk for various late effects
- Describe screening tests to better monitor for various late effects childhood cancer survivors
Instructions for CME Credit:
The estimated time to review the material and answer the self-assessment questionnaire is listed at the beginning of each module. Record your answers to the self-assessment questionnaires and complete the program evaluation on the form provided and submit it online. A score of at least 80 percent is required for CME credit. Follow the instructions at the end to print a CME certificate. The CME must be completed in a single session.
Hardware and Software Required to Complete This Activity:
For this CME program, the minimum web browser requirements are Internet Explorer 7.0, Firefox 3.0 or Google Chrome 2.0. A broadband internet connection is recommended (and required for video CMEs). For dialup access, a 56 kbit modem is recommended.

Lillian Meacham MD
Dr. Meacham is a board certified pediatric endocrinologist, Professor of Pediatrics at Emory University and has been medical director of the Cancer Survivor Program of the Aflac Cancer Center since its inception in 2001. She and her team provide survivorship care to over 900 patients. She is currently the coordinator for the Georgia Pediatric Alliance for Cancer Survivors a collaboration of the all the pediatric oncology long-term follow-up programs in Georgia. She has lectured extensively about SurvivorCare throughout Georgia, the United States and abroad. Her role in SurvivorLink™ includes assisting in the development of the educational material for healthcare providers and providing lectures to state based healthcare professional organizations like the American Academy of Pediatrics and Academy of Family Physicians.
